Assistance Request Form

Marie’s Community Distribution will be open for curbside pick up of essential items on Tuesdays and Thursdays beginning February 23, 2021.  Complete the form below by noon on the day before to request assistance for distribution between 10am – 2pm on Tuesdays and Thursdays or call 812-941-0563.
