Supported Living Program Assistant Director

Full Time, Non-Exempt

REPORTS TO: Supported Living Program Director

SUPERVISES: Direct Support Professionals (DSP)

PRIMARY FUNCTION: Supervise and assist the DSPs and the SLP consumers. The majority of the position will be fulfilled by providing direct client care; the remainder of the time will be spent on administrative duties. 

MAJOR RESPONSIBILITIES AND REGULAR ACTIVITIES

DIRECT CLIENT CARE

  1. SLPA Director will meet with their consumers on a schedule that uses the authorized NOA hours.
  2. The SLPA Director will ensure that services being provided to the consumer matches the goals outlined in his/her Individual Service Plan (ISP) responsibilities
  3. SLPA Director will utilize the electronic visit verification and maintain all required service documentation associated with direct care service hours.
  4. SLPA Director will maintain an accurate log of services which will reflect his/ her time for service, along with the goal addressed and description of activity spent.
  5. SLPA Director will routinely communicate with team members; specifically to SLP Director and DSPs in an effort to provide quality and continuity of service to the consumer.
  6. SLPA Director will complete a daily goal sheet for each consumer that was provided services that day and that record is to be maintained in the consumer’s binder and submitted at the end of the month.
  7. SLPA Director will notify SLP Director and/or Social Services Director as soon as possible upon learning that he/she will not be able to meet with his/her consumer.
  8. SLPA Director will be conscientious in scheduling and keeping appointments with his/her consumer. This includes arriving on time for scheduled appointments, transporting a consumer to his/her scheduled medical appointments in a timely manner and canceling an appointment with a consumer prior to the scheduled meeting time.
  9. SLPA Director will participate in an initial training as well as ongoing trainings and in-service meetings as required by the State of Indiana, and/or as deemed necessary by SLP Program Director and/or Director of Social Services.

 

 ADMINISTRATIVE DUTIES

  1. The SLPA Director will conduct monthly home visits, and maintain and update consumers’ in-home binders.
  2. The SLPA Director will complete all required program forms and maintain them in the agency files.
  3. The SLPA Director will maintain and update DSP’s materials and consumer binders.
  4. On occasion the SLPA Director will review bi-monthly time reports submitted by SLPA staff. The SLPA Director will review the reports for accuracy before submitting to the Accountant.
  5. The SLPA Director will assist in developing the training material in accordance with SLP guidelines for all SLP staff. The SLPA Director will also assist in conducting the training sessions in accordance with required training schedules.
  6. The SLPA Director will partner with the director for on call assistance. On call is rotated among four staff members for support. During vacations or crisis times assistance with on call may be requested.
  7. Any other duties to be assigned by SLP Director.

 

SKILLS, KNOWLEDGE, AND/OR ABILITIES

  1. Maintain his/her driver’s license and automobile insurance current, and will provide a copy of each to be kept in his/her employee file.
  2. Pass random drug screens
  3. One year working with individuals with intellectual and developmental disabilities.
  4. Ability to build positive relationships and welcomes diversity.
  5. Provide guidance, plan, support and assist with the implementation of set goals.
  6. Must be organized, a problem solver and the ability to anticipate future needs.
  7. This position requires someone with strong emotional intelligence and professionalism.

EDUCATION, TRAINING AND/OR EXPERIENCE

  1. Successfully fulfill all of the required trainings outlined on the New Employee Orientation.
  2. Ability to use Microsoft Word, Excel and have internet competency.
  3. Bachelor Degree or Associate Degree with equivalent work experience.

WORKING ENVIRONMENT

  1. The SLPA Director will work in office, and will provide in-home services.
  2. Travel within Floyd and Clark Counties will be required for the SLPA Director, therefore reliable transportation is a requirement.
